New Student Forms
The following items must be submitted at the time of registration:
- New Student Enrollment Form NEW STUDENT ENROLLMENT
- Student Housing Questionnaire - STQ Required
- Oral Health Assessment ORAL HEALTH ASSESSMENT
- Immunization records
- Birth Certificate/Passport (originals) - office will make copies
- Proof of Residency (utility bill -gas/electric)
- Address verification may occur with home visits
